If you have issues related to your Corporation or LLC please contact the Corporations Division at 602-542-3026.

Alternatively, if you have problems submitting the form or have questions, you may contact the Commission’s designated employee representative, Carolyn Buck, at 602 542-2237.

Set forth below are the statutory provisions that have been enacted to define “small business” and establish the SMALL BUSINESS BILL OF RIGHTS. The Commission’s authority applies to several different areas of regulation, such as the incorporation of business organizations, securities sales and registration, and the oversight of public service corporations that provide utility services. Because of the Commission’s broad range of regulatory actions, not all of the provisions in the SMALL BUSINESS BILL OF RIGHTS may apply to your particular circumstances. If you have questions, please feel free to contact the designated employee representative listed above.
